The Assurant Health small group insurance portfolio was designed to address the different needs of each small business. Real Choices has three medical insurance plans, two Health Savings Account (HSA) plans and two different Health Reimbursement Arrangement (HRA) plans. Employers can easily make their group insurance plan suitable for their employees when so many choices are offered.
For your convenience, a brief overview of the medical insurance and ancillary coverages are given below.
Real Choices I
Real Choices I offers employers the most extensive coverage. With nine deductible choices that range from $0 to $10,000, the plan is sure to please the established business owner. Additional benefit highlights include:
- Up to $10 million lifetime maximum coverage
- Attractive prescription drug copays
- First-dollar preventive care benefits
- Broad choices for office visit copays for primary care physician
Real Choices II
Real Choices II is extremely popular because it offers the most choices. Nine deductible choices and five benefit percentage choices make this plan one that can be designed to fit the employees’ unique needs. Other benefit highlights include:
- $2 million lifetime benefit maximum included
- First-dollar preventive care benefit included
- First-dollar diagnostic x-ray and lab services benefit included
- Five optional prescription drug benefit choices
Real Choices III
Real Choices III is designed for the employer with a new business or one that has no health insurance. Real Choices III is our most affordable plan and is available without having quality coverage compromised. Benefit highlights include:
- Optional first-dollar benefit for physician office visits
- $1 million lifetime benefit maximum
- First-dollar preventive care benefit
- Optional first-dollar accident medical expense benefit
Real Choices Health Reimbursement Arrangement (HRA) and Health Savings Account (HSA)
Responding to the increasing popularity of consumer-driven health plans, Assurant Health has designed two plan choices for HRAs and two qualified high deductible health plan choices for HSAs. These tax favored plans open a wide range of choices for both the employer and the employees. Benefit choices include:
- Wide range of deductible and benefit percentage choices
- First-dollar preventive care benefit included
- Optional first-dollar accident medical expense benefit
- Lifetime maximum benefit from $2 million to $10 million
Real Choices Indemnity Plans
Indemnity plans offer your employees complete freedom to choose doctors and hospitals. These plans are best suited for those who live and work outside a PPO network area.
Real Choices Ancillary Plans
Real Choices ancillary coverage includes life insurance, short-term disability insurance and dental insurance. These attractive benefits can be added to your medical plan and you’ll receive a premium discount on the ancillary premium just for adding one or more of the following:
Life Insurance Each employee qualifies for a guaranteed issue amount of life insurance (amount depends on medical plan selected).
Short Term Disability You can select coverage up to 66% of an employee’s weekly salary. Two plans are available—amount is based on a percentage of employee’s salary or a flat amount from $50 to $1,000 per week.
Dental Insurance Select a plan that best suits your needs! Real Choices dental insurance has six plans available. You can enjoy first-dollar coverage for preventive services with no deductible or coinsurance to satisfy! Dental insurance can also be purchased as a stand-alone product for groups of three or more.
