Ancillary Health Services are services provided to patients to aid in the diagnosis or treatment of an illness or injury. They may be either diagnostic or therapeutic in nature. Examples of Diagnostic Ancillary Health Services include: laboratory services; radiology; magnetic resonance imaging (MRI), etc. Examples of Therapeutic Ancillary Health Services include: durable medical equipment; home health care; home infusion therapy; physical therapy; specialty pharmacy services; speech therapy; surgical centers; transplant services, etc.
Retail Health Clinics are a new type of Ancillary Health Service added to this Web page for your convenience. Retail Health Clinics are small health care facilities located in high-traffic retail outlets with pharmacies that provide affordable and accessible, non-emergency health care to consumers who otherwise would have to wait for appointments with a traditional primary care physician or provider (PCP). Often open seven days a week, Retail Health Clinics provide a health care option that is intended to compliment, not replace, traditional medical service providers. Retail Health Clinics are primarily staffed by highly qualified advanced practice nurses (including nurse practitioners (NP) and clinical nurse specialists (CNS)), or by physician assistants (PA). Retail Health Clinic staff receive advanced education in providing quality health care for common episodic ailments, including cold/flu, rashes/skin irritation, and muscle strains or sprains.
Certain Ancillary Health Services may not be available within your primary PPO network. This has prompted Assurant Health to contract directly with the providers listed below.

Lab Card Select is a voluntary outpatient laboratory testing program that allows individuals to take control of their health care decisions. When visiting your doctor’s office, show your Assurant Health Id card with the Lab Card Select logo and request that your specimens be sent under the Lab Card Select Program to a contracted Quest Diagnostic laboratory for testing.
If your doctor customarily collects specimens in the office, the office simply calls Lab Card Select client services at
